CNSC - Online Appointment Form

REMINDERS

This is the Online Appointment Form of Camarines Norte State College. This form is to be used by students and guests of the CNSC and its satelite campuses to control the number of people entering the different campuses everyday to prevent the further spread of the Corona Virus Diseases 2019 (CoViD 19).

Walk-in clients will not be allowed starting on the aforementioned date. This online form shall also serve as a Digital Logbook / Contact Tracing Form for Camarines Norte State College.

At Camarines Norte State College, we value your Data Privacy and in compliance with RA 10173 or the Data Privacy Act of 2012, this form will be collecting the following information:
